No notarization, 2 pages, medical insurance card must be attached.
This form is required for all students participating in youth travel or overnight events.

There will be some forms that require a notary. Marji Monroe, our morning receptionist, is a notary public. Bring your forms up to the church office in the morning from 9:00am - 12:00pm and she can notarize them.
